Balance Point is located on the west side of Indianapolis. From I-465, take exit 17 (West 38th Street). Drive west on 38th Street to Shore Drive, and turn north (right). Take Shore Drive to Parkdale Place, and turn east (right). Balance Point is located at 6640 Parkdale Place, Suite O.

Before your first appointment, we’d appreciate you filling out a few forms. Just download them by clicking on the "Patient Intake Forms" link below. Then print all the pages, complete the forms, and bring them with you to your first scheduled visit. Please read and follow the pre-testing instructions included in your packet.
